REQUEST FOR QUOTATION
Shopping for Goods

The Government of the Republic of the Philippines has received a loan from the World Bank towards the cost of the Philippine Rural Development Project (PRDP) and it intends to apply part of the proceeds of this loan to payment for the cost of:

ESTABLISHMENT OF ARTIFICIAL REEFS – CORALS RECRUITMENT (SUPPLY, DELIVERY AND INSTALLATION OF ARTIFICIAL REEFS)

INSTRUCTIONS:
1. Bidders are required to read the instructions and fill all the blanks properly.
2. Prospective Bidders/Suppliers may submit their own canvass form in accomplishing their bid proposals/quotations subject to the condition that the official canvass form shall be filled up (with the offered brand, unit price and total price), signed properly and attached together with the Bidder’s/Supplier’s own canvass form. (Please write legibly).
3. Any specification other than those required/stated in this form shall not be considered in the evaluation of bids.
4. Quotation/s must include all kinds of taxes for the item(s)/services listed hereunder, including delivery charges.
5. Indicate the brand name/model of the offered item(s) or product(s).
6. Price quotation/s submitted shall be valid THIRTY (30) days from the deadline of submission of quotations.
7. Terms of Payment within - Thirty (30) days from delivery of item/s.
8. All Request for Quotations (RFQs) shall be submitted through:
a. Sealed Envelope and hand-carry to the Office of the BAC at the General Services Office, Barangay II, Roxas, Palawan
9. Awarding shall be made on: [____] item [___] per lot [__√__] per package. Award shall be made to the lowest evaluated quotation that complies with the minimum technical specifications and other terms and condition state herein.

10. DEADLINE FOR THE SUBMISSION OF BIDS: November 17, 2021 at 10:00AM
11. Validity of stocks: 60 days
12. Delivery period: Sixty (60)days upon receipt of approved Purchase/Work Order.

1. All quotation must be marked with the name of the contract and the Solicitation No. PRDP-MPA-R04B-PAL-001-ROX-001-2015. All entries must be typewritten or hand written eligibly.

4. Quotations must be submitted not later than November 17, 2021 at 10:00AM.

5. The procurement procedures will be conducted in accordance with the provisions of the World Bank Guidelines and taking into consideration the related provisions in the Project Loan Agreement and Guidelines in the Procurement under the IBRD loans and IDA credits.

6. The MLGU of Roxas, Palawan reserves the right to accept or reject any quotation and to annul the procurement process or reject all bids at any time prior to contract award, without thereby incurring any liability to the affected bidder/s.
