Failure to comply/submit with any of the following requirements shall be a basis for disqualification: 1. Requirements shall be attached upon submission of the quotation: • Certified True Copy of DTI Certificate of Business Name Registration / SEC Registration Certificate, and Articles of Incorporation. • Certified True Copy of Mayor's / Business Permit • Certified True Copy of Tax Clearance per Executive Order 398 Series 2005, as finally reviewed and approved by the BIR (for ABC's above Php500,000.00) • Certified True Copy of Latest Annual Income Tax Return with supporting documents: (for ABC's above Php500,000.00) • Certified True Copy of Certificate of PHILGEPS Registration / Number (Red or Platinum Membership). • Omnibus Sworn Statement in accordance with Section 25.3 of the revised IRR of RA 9184. • Notarized SPA or Secretary’s Certificate if the Bidder is an Authorized Representative. 2. All quotations/entries must be computerized/handwritten. 3. All quotations shall be considered as fixed prices and not subject to price escalation during contract implementation. 4. Duly signed Price Form or Itemized Price Schedule and List of Goods or Bill of Quantities. 5. Any erasures or overwriting shall be valid only if they are signed or initialed by the Contractor/Supplier or his/her authorized representative. The quotation and eligibility documents must be submitted in a sealed envelope on or before December 11, 2025 12:00 PM at the Procurement Office, NEUST, Gen. Tinio St. Campus, Cabanatuan City. Section 39 of RA No. 9184, requires the winning bidder to post a performance security, in such form and amount specified under Section 39 of the 2016 Revised IRR, prior to the signing of the contract as a measure of guarantee for the faithful performance of and compliance with his obligations under the contract prepared in accordance with the Bidding Documents The NEUST reserves the right to reject any or all Quotations, to waive any minor defects therein, to reject all Quotations at any time prior to contract award, without thereby incurring any liability to the affected Bidder(s), and to accept only the offer that is compliant with the law and requirements of the procuring entity. The NEUST assumes no responsibility whatsoever to compensate or indemnify Bidders for any expenses incurred in the preparation of their Quotation.
