Description   Request for Proposal The Bureau of Fisheries and Aquatic Resources Regional Office No. 5 (BFAR RO 5), Fabrica, Bula, Camarines Sur is requesting from prospective suppliers to submit written proposal for the Supply and Delivery of Bangus Fingerlings for Techno-Demo Projects for Bangus Culture under SAAD Phase II. Quantity Unit Item Description Unit Cost Total Cost 20,000 Piece Bangus Fingerlings (4-5” TL) 7.00 140,000.00 Conditions: 1. To be delivered within thirty (30) calendar days upon receipt of the Purchase Order by the supplier in Capalonga, Camarines Norte. 2. Deliveries must be early in the morning or late in the afternoon. 3. Mortality during deliveries must be replaced immediately. Together with the written proposal, the participating prospective suppliers shall also submit the following requirements: 1. Mayor’s/Business Permit 2. PhilGEPS Registration No. 3. Omnibus Sworn Statement 4. Income/Business Tax Return (For ABCs above 500K) The Approved Budget for the Contract (ABC) is One Hundred Forty Thousand Pesos (PhP140,000.00) only. All written proposals shall be submitted to the Office of the BAC Secretariat, BFAR 5, Fabrica, Bula, Camarines Sur before 1:00 o’clock in the afternoon of February 26, 2024, which shall be opened and evaluated by the BAC thereafter at the Office of the BAC Secretariat at the same address. This requisition is made thru Alternative Mode of Procurement under Rule XVI of the 2016 Revised IRR of RA 9184, specifically Small-Value Procurement under Section 53.9, as per prior recommendation of the Bids and Awards Committee.
