Description   Request for Proposal The Bureau of Fisheries and Aquatic Resources Regional Office No. 5 (BFAR RO 5), Fabrica, Bula, Camarines Sur is requesting from prospective suppliers to submit written proposal for the Meals during the conduct of the Orientation/Training for the Beneficiaries of Seaweeds Nursery on February 15-16, 2024 in Dimasalang, Masbate. Quantity Unit Item Description Unit Cost Total Cost Pax 42 3 meals and 2 snacks for 2 days 600.00 50,400.00 Conditions: 1. To be served on February 15-16, 2024 in Dimasalang, Masbate. 2. With unlimited drinking coffee with creamer and steamed rice during meal. Together with the written proposal, the participating prospective suppliers shall also submit the following requirements: 1. Mayor’s/Business Permit 2. PhilGEPS Registration No. 3. Omnibus Sworn Statement 4. Income/Business Tax Return (For ABCs above 500K) The Approved Budget for the Contract (ABC) is Fifty Thousand Four Hundred Pesos (PhP50,400.00) only. All written proposals shall be submitted to the Office of the BAC Secretariat, BFAR 5, Fabrica, Bula, Camarines Sur before 1:00 o’clock in the afternoon of February 12, 2024, which shall be opened and evaluated by the BAC thereafter at the Office of the BAC Secretariat at the same address.
