Description   QTY. Unit DESCRIPTION 17333 kgs Collection and Transport of Hazardous / Infectious Wastes of the Laoag City General Hospital (CY - 2023) Highlights of Hauling Services: 1. Training and Orientation of Utility Workers on proper segregation and safe handling of hazardous waste. 2. Compliance on DENR-EMB and DOH Guidelines and Accreditation. 3. Issuance of transport manifest in every collection with proper labeling and tagging. 4. Provide drums/ med bin and exchange of med bin container every collection to avoid cross contamination. 5. Every two (2) weeks hazardous waste collection as per DOH Manual and Standard. 6. Issuance of Certificate of Final Disposal with DENR received copy. 7. DENR-EMB Accreditation documents. 8. Well trained personnel who will handle hazardous waste transport procedures. 9. Assistance in DENR application on Permit to Transport and the likes. 10. One (1) year contract of hauling service. 11. Must be an Integrated Treatment Facility that can treat various industiral hazardous waste such as, but not limited to: a. Pathological and infectious waste (M501 b. Wastes with inorganic chemicals (D401-D405, D408, D499) c. Busted fluorescent lamps (D407) d. Waste organic solvents (G703-G704) e. Grease wastes (H802) f. Waste Oil (1101-1102) g. Must be a registered Transporter and Treater under RA6969 13. Must have 10 years experience in the field of service as Treater and treater 14. Must be ISO-certified company, particularly ISO14001 (Environmental Management Systems), ISO9001 Quality Management Systems, and ISO45001 Manangement Systems of Occupationl Health and Safety) 15. Must have a partnetship with a Category 4 Sanitary Engineered Landfill 16. Bids received in excess of the ABC shall be automatically rejected at bid opening. The LAOAG CITY now invites bids for the Cost of collection and transport of 17,333 kgs. Hazardous/Infectious Wastes of Laoag City General Hospital. A complete set of Bidding Documents may be purchased by interested Bidders on January 26 – February 16, 2024 from the address below and upon payment of a nonrefundable fee for the Bidding Documents in the amount of P5,000.00 or P500.00 per line item. The LAOAG CITY will hold a Pre-Bid Conference on February 5, 2024, 3:00 PM at City Conference Room, City Hall Annex, Laoag City, which shall be open only to all interested parties who have purchased the Bidding Documents. Bids must be delivered to the address below on or February 16, 2024, 3:00PM. All Bids must be accompanied by a bid security in any of the acceptable forms and in the amount stated in ITB.
