Description   Request for Proposal The Bureau of Fisheries and Aquatic Resources Regional Office No. 5 (BFAR RO 5), Fabrica, Bula, Camarines Sur is requesting from prospective suppliers to submit written proposal for the Printing and Delivery of Waiver Forms for Fuel Assistance for Fisherfolk 2023. Quantity Unit Item Description Unit Cost Total Cost 300 Pad Waiver Forms 350.00 105,000.00 Colored; 50/3; Legal size; carbonized (Bond white, onion skin pink, onion skin yellow) with control number Conditions: 1. To be delivered within fifteen (15) calendar days upon receipt of the Purchase Order by the supplier at BFAR RO5, Fabrica, Bula, Camarines Sur. 2. Delivered forms must be free from defects. Together with the written proposal, the participating prospective suppliers shall also submit the following requirements: 1. Mayor’s/Business Permit 2. PhilGEPS Registration No. 3. Omnibus Sworn Statement 4. Income/Business Tax Return (For ABCs above 500K) The Approved Budget for the Contract (ABC) is One Hundred Five Thousand Pesos (PhP105,000.00) only. All written proposals shall be submitted to the Office of the BAC Secretariat, BFAR 5, Fabrica, Bula, Camarines Sur before 1:00 o’clock in the afternoon of January 29, 2024, which shall be opened and evaluated by the BAC thereafter at the Office of the BAC Secretariat at the same address. This requisition is made thru Alternative Mode of Procurement under Rule XVI of the 2016 Revised IRR of RA 9184, specifically Small-Value Procurement under Section 53.9, as per prior recommendation of the Bids and Awards Committee.
