Description   Request for Proposal The Bureau of Fisheries and Aquatic Resources Regional Office No. 5 (BFAR RO 5), Fabrica, Bula, Camarines Sur is requesting from prospective suppliers to submit written proposal for the Supply and Delivery of Bangus Fry at MDFFN Daang Lungsod, Mobo, Masbate. Quantity Unit Item Description Unit Cost Total Cost 500,000 Piece Bangus Fry (hatchery bred, 1-2.5 cm length) 0.62 310,000.00 Conditions: • To be delivered within twenty (20) working days upon receipt of the Purchase Order by the supplier at MDFFN Daang Lungsod, Mobo, Masbate • It should be live fry not dying • Locally produced fry not imported • With 10% mortality allowance Together with the written proposal, the participating prospective suppliers shall also submit the following requirements: 1. Mayor’s/Business Permit 2. PhilGEPS Registration No. 3. Omnibus Sworn Statement 4. Income/Business Tax Return (For ABCs above 500K) The Approved Budget for the Contract (ABC) is Three Hundred Ten Thousand Pesos (PhP310,000.00) only. All written proposals shall be submitted to the Office of the BAC Secretariat, BFAR 5, Fabrica, Bula, Camarines Sur before 1:00 o’clock in the afternoon of January 29, 2024, which shall be opened and evaluated by the BAC thereafter at the Office of the BAC Secretariat at the same address. This requisition is made thru Alternative Mode of Procurement under Rule XVI of the 2016 Revised IRR of RA 9184, specifically Small-Value Procurement under Section 53.9, as per prior recommendation of the Bids and Awards Committee.
